Adoption Profiles

Our online Volunteer forms will be available shortly. In the meantime, please download the Volunteer Profile form specific to the animal you are interested in adopting, and either fax it back to us at (262) 723-1001 or mail it to P.O. Box 1000, Elkhorn, Wisconsin 53121. You may fill out the form and bring it back to the Shelter during normal business hours.
