DOG ADOPTION APPLICATION INFORMATION

Thank you for your interest in adopting from Lakeland Animal Shelter.  We hope we can be of assistance to you in finding a Dog suitable for your lifestyle.  In order to fill out our online application, you will need to have the following information with you:

Name and number of your landlord if you rent

Name and number of any veterinarians you have used in the past 5 years

 Before completing your application, please take the time to read the information below regarding our adoption policies and fees.  Information submitted to Lakeland Animal Shelter is used for processing of our application only and is not given or sold to any marketing agency.  Please allow some time to fill out this application and make sure that it is complete, as incomplete applications cannot be approved.

The pets available for adoption came here from a variety of sources, both stray and unwanted.  They are examined by a staff member upon entry and their health is routinely monitored while at the shelter.  However, there is always a chance that a cat or dog is incubating a disease without showing any clinical signs.  This is true of any pet anywhere, whether  breeder, private owner, or animal shelter.

The benefits of adopting a dog from the Lakeland Animal Shelter are numerous.  Sterilization and other veterinary services are included in our low adoption fee.  The following chart outlines the benefits of adopting a dog from the Lakeland Animal Shelter

Staff and volunteers are available to answer questions about our current resident cats and dogs, specific breeds, obedience and crate training, and other areas of pet relationships.  Please contact us if you need additional information.

 Our Adoption process is very simple.  Our requirements include:

·       An adoption application completed and signed by a person over 18 years of age.  When applying online, your signature will be obtained and your identification will be verified at a later point in the adoption process.

·        Photo identification with an address that matches the address on your application.

·        Verification that animals are allowed in your home.

·        If you rent:  Your landlord’s name and address and a copy of your lease if available

·        If you own your home:  Proof of ownership (a deed, mortgage paper, payment book, etc.).

·        Proof that companion animals currently living in your home are up-to-date on rabies and distemper vaccinations or your veterinarian’s name and telephone number

We will not knowingly place a pet in a situation where its well-being is questionable.  Therefore, we reserve the right to refuse an adoption application.  No pet will be adopted to prospective owners who mislead or fail to provide accurate information on this adoption application.

ADOPTION PROCEDURE

 ·        Fill out adoption application and submit to Lakeland Animal Shelter

·         There is a mandatory 24 hour waiting period once an adoption application has been turned in.

·         Please call the Lakeland Animal Shelter at 723-1000 after 2:00 PM the next business day (Monday-Friday) after turning in your adoption application.  At that point a shelter staff member will get any additional information required of you in order to approve or deny your application.  Due to the number of incoming calls and the time spent in checking applications, we are unable to contact each person that has completed an adoption application.  It is your responsibility to call the shelter the next day.  If we do not hear from you, then it is assumed that you are no longer interested in adopting and your application will be filed.  (Please realize that at times we do receive several adoption applications on a single dog .  When this happens we may eventually approve all of the applications on the dog, but the prospective adopters with the first approved application on the dog will be given first choice on adopting the dog.  Please understand that this type of situation is difficult for everyone involved and we try to make a fair decision based on giving preference to the adoption application that first meets the adoption requirements listed on the previous page.)

·        Once your application has been approved, arrangements will be made to finalize the adoption. 

·      If the dog that you have chosen to adopt is not already altered, the spay or neuter will need to be completed before you can take the dog home.  The shelter will make arrangements to have the surgery performed as soon as possible and arrangements will be made for you to pick the animal up after surgery, either at the shelter or the veterinarian's office.  If picking up from the veterinarian's office,  you will need to return to Lakeland Animal Shelter to complete the adoption contract and pay the adoption fee.  You will be given a receipt that must be presented at the veterinarian's office in order for the dog to be released.

·        If the dog you have chosen is already spayed/neutered, then you will be able to complete the adoption contract and pay the adoption fee on the appointed day you pick up the dog at Lakeland Animal Shelter.

PLEASE READ THE FOLLOWING VERY CAREFULLY & ENTER YOUR INITIALS IN THE BOX  IF YOU UNDERSTAND & AGREE TO ABIDE BY THEM:

      If the dog/puppy you have chosen is already altered or is too young to be altered at the time of adoption, you have seven days to take it to the veterinarian of your choice for a  complete physical exam and required shots, i.e., rabies vaccination.  The cost of the exam/shots is your responsibility.  You will be given a copy of the pet’s vaccination/medical records from the shelter to show your veterinarian.  If you fail to do so, the shelter will not be responsible for any  undetected health issues.

       ALL DOGS WILL BE ALTERED PRIOR TO ADOPTION UNLESS  TOO YOUNG.  IN THAT CASE, THE PET MUST BE ALTERED ON OR BEFORE THE DATE SPECIFIED IN THE ADOPTION CONTRACT

      According to Wisconsin Statute, and the statutes in many other states, all dogs are required to be vaccinated against rabies and licensed.  They must wear proof of vaccination and licensing on their collars at all times.  There are also annual shots that must be given.  Please talk to your veterinarian and show him/her the medical records from the shelter

    Do you fully understand that any pet adopted from us cannot be sold or given to any person or agency?  If you are unable to keep the adopted pet, it must be returned to this shelter.  The only exception to this policy is if the pet is humanely euthanized by a licensed veterinarian if the situation requires this procedure.

  IIt is illegal to allow a dog to run loose at any time in the State of Wisconsin (and in most other states), including in rural areas.  If your dog is caught running loose, you will receive a citation ticket and pay a fine.

WE WILL NOT KNOWINGLY PLACE A PET IN A SITUATION WHERE ITS WELL BEING IS QUESTIONABLE.  THEREFORE, WE RESERVE THE RIGHT TO REFUSE AN ADOPTION APPLICATION.  NO PET WILL BE ADOPTED TO PROSPECTIVE OWNERS WHO MISLEAD OR FAIL TO PROVIDE COMPLETE AND ACCURATE INFORMATION ON THIS ADOPTION APPLICATION.
