Before completing your application, please take the time to read the information below regarding our adoption policies and fees. Information submitted to Lakeland Animal Shelter is used for processing of our application only and is not given or sold to any marketing agency. Please allow some time to fill out this application and make sure that it is complete, as incomplete applications cannot be approved.
You must be at least 18 years of age to adopt from our shelter. Applications completed by persons under 18 years of age will not be considered.

The pets available for adoption came here from a variety of sources, both stray and unwanted. They are examined by a staff member upon entry and their health is routinely monitored while at the shelter. However, there is always a chance that a cat or dog is incubating a disease without showing any clinical signs. This is true of any pet anywhere, whether breeder, private owner, or animal shelter.

Staff and volunteers are available to answer questions about our current resident cats and dogs, specific breeds, obedience and crate training, and other areas of pet relationships. Please take your time; walk through the cat room and read the comments on the cards attached to the cages.
Our Adoption process is very simple. Our requirements include:
· A completed adoption application.
· Photo identification with an address that matches the address on your application. Identification must also show that you are 18 years of age or older. This identification will be required in order to pick up an animal.
· Verification that animals are allowed in your home.
· If you rent: Your landlord’s name and address and a copy of your lease if available
· If you own your home: Proof of ownership (a deed, mortgage paper, payment book, etc.).
· Proof that companion animals currently living in your home are up-to-date on rabies and distemper vaccinations or your veterinarian’s name and telephone number.
We will not knowingly place a pet in a situation where its well-being is questionable. Therefore, we reserve the right to refuse an adoption application. No pet will be adopted to prospective owners who mislead or fail to provide accurate information on this adoption application.
ADOPTION PROCEDURE
· Fill out adoption application and submit to Lakeland Animal Shelter
· There is a mandatory 24 hour waiting period once an adoption application has been turned in.
· Please call the Lakeland Animal Shelter at 262-723-1000 after 2:00 PM the next business day (Monday-Friday) after turning in your adoption application. At that point a shelter staff member will get any additional information required of you in order to approve or deny your application. Due to the number of incoming calls and the time spent in checking applications, we are unable to contact each person that has completed an adoption application. It is your responsibility to call the shelter the next day. If we do not hear from you, then it is assumed that you are no longer interested in adopting and your application will be filed. (Please realize that at times we do receive several adoption applications on a single cat . When this happens we may eventually approve all of the applications on the cat, but the prospective adopters with the first approved application on the cat will be given first choice on adopting the cat. Please understand that this type of situation is difficult for everyone involved and we try to make a fair decision based on giving preference to the adoption application that first meets the adoption requirements listed on the previous page.)
· Once your application has been approved, arrangements will be made to finalize the adoption.
· If the cat that you have chosen to adopt is not already spayed/neutered the spay or neuter will need to be completed before you can take the cat home. The shelter will make arrangements to have the surgery performed as soon as possible and arrangements will be made for you to pick the animal up after surgery.
· Before you pick up the cat at the veterinarian’s office, you will need to return to Lakeland Animal Shelter to complete the adoption contract and pay the adoption fee. You will be given a receipt upon payment and you will take the receipt with you when you pick up the cat at the veterinarian’s office.
· If the cat you have chosen is already spayed/neutered, then you will be able to complete the adoption contract and pay the adoption fee on the appointed day you pick up the cat at Lakeland Animal Shelter.

If the cat you have chosen is already altered, or is too young to be altered at the time of the adoption, you have 7 days to bring your cat to a veterinarian for a physical exam and any required shots, including rabies. The cost of the exam and shots is your responsibility.
ALL PETS MUST BE ALTERED PRIOR TO ADOPTION. IF THE PET IS TOO YOUNG, IT MUST BE ON OR BEFORE THE DATE SPECIFIED IN THE ADOPTION CONTRACT.
Where required by law, cats must be vaccinated and licensed every year and wear the tags on a collar.
Do you fully understand that any pet adopted from us cannot be sold or given to any person or agency? If you are unable to keep the adopted pet, it must be returned to this shelter. The only exception to this policy is if the pet is humanely euthanized by a licensed veterinarian if the situation requires this procedure.
It is illegal in some municipalities for cats to run loose. If you let your cat outdoors, you will receive a citation and have to pay a fine.
WE WILL NOT KNOWINGLY PLACE A PET IN A SITUATION WHERE ITS WELL-BEING IS QUESTIONABLE. THEREFORE, WE RESERVE THE RIGHT TO REFUSE AN ADOPTION APPLICATION. NO PET WILL BE ADOPTED TO PROSPECTIVE OWNERS WHO MISLEAD OR FAIL TO PROVIDE COMPLETE AND ACCURATE INFORMATION ON THIS ADOPTION APPLICATION.
